The two forms to the right are the first two you should fill out if you are a new patient seeing Dr. Perryman for the first time.

HIPPA is a form for authorization of release of identifying health information.

Patient History is somewhat self-explanatory; it provides us with vital information regarding your medical history.

NEW AND EXISTING PATIENTS -- RELEASE FORMS

The form to the right is a record release form for Dr. Perryman. This form enables us to secure vital information from your previous or existing eye doctor.
